The Portal allows you to review and maintain your health records, access office instructions and educational literature, and securely communicate with your physicians and administrative staff. The Patient Portal is a KEY COMPONENT of our Electronic Medical Records system. The very nature of our practice generates large volumes of communication and exchange of information among practitioners, patients, staff, laboratories, pharmacies and a wide range of other collaborating health care individuals and organizations. The Secure Patient Portal facilitates clear and efficient exchange of information and responses to your needs and questions. Because this Portal is integrated with your medical record, we can easily track our communications and incorporate our exchanges into your chart. New Patients When you are scheduled for your first office visit as a new patient, we provide you with instructions for loging into the Patient Portal. You will be directed to login and complete your Health Record. This includes all of your demographic information, medical history, medications, pharmacies, other physicians and insurance information. It is extremely important that youcomplete these areas of your Health Record prior to you initial visit. Established Patients Please call the office and we will give you instructions for logging into your Portal. You will be using your Portal for all routine questions about your care, prescriptions, lab tests and appointments. You should review and update all of your personal information, lists of medicines & allergies, other physicians and providers, and overall health status.
