This form is a brief summary of your health concerns that allows us to have a focused and productive telephone conversation with you about becoming a patient . The information you provide is extremely helpful in our understanding your needs and expectations. Within two business days of receiving your form, we will contact you answer all your questions, get a good understanding of your health concerns, and determine the best match of providers and schedules.
The Prospective Patient Summary is an Adobe AcrobatTM file that you need to download, print, complete and either fax to us at 301.330.6515, or mail to us at Optimal Health Physicians, 15235 Shady Grove Rd., Suite 102, Rockville, MD 20850. Please do not attach the form to an email to us. Email messages are not secure and should never be used to communicate private health information. If you have any difficulty accessing, printing or completing the Prospective Patient Summary, please call our office at 301.330.9430 betwen the hours of 9am-1pm, and 2pm-5pm.
We Do Not Participate in Insurance Plans or Medicare
Due to the time and complexity inherent in diagnosing and treating our patients' often highly challenging health issues we have been forced to "opt out" of the insurance industry's constrains that severely limit our diagnostic and therapeutic options.We do of course provide our patients with the documentation they need to submit claims to their health plans for reimbursed of the "out-of-plan" services we render. The degree to which insurance plans reimburse our patients is purely a matter of each particular plan's benefits and coverage. Medicare-eligible patients are required to sign a private contract with OHP for care, and are not permitted to submit claims to the Medicare program.
